From the New Jersey Department of Health:
If you experience an error or need support with the New Jersey vaccine scheduling system, you can complete and online form at: covid19.nj.gov/vaccinehelp
In addition, you can call 855-568-0545. Live operators are available for phone support regarding vaccine applications and scheduling.
